About This Grant:
Penobscot Community Health Center (PCHC) seeks to expand previous efforts that focused on introducing and/or expanding the implementation of priority evidence-based interventions to increase CRC screening rates at health clinics. The previous MCF-funded project increased screening rates by 1.5-3% at the Adult Wellness Center and Hope House by providing education, outreach, improved documentation, transportation support, and introducing Cologuard as an alternative to colonoscopies at Hope House. Through this current initiative, PCHC endeavors to expand these successful initiatives to additional practice sites.
