Expanding Systems to Increase Colorectal Cancer Screening through Patient Outreach and Recall

About This Grant:

Penobscot Community Health Care (PCHC) will expand our colorectal cancer prevention efforts to Seaport Community Health Center in Belfast.  We will hire a patient navigator medical assistant who will work closely with PCHC clinical leadership and Seaport providers to adopt processes and procedures to improve colorectal cancer screening rates.  The patient navigator will utilize a population health management approach that involves outreach and education.  While this project will focus on colorectal cancer screening, the navigator will also help identify those who are due for breast and cervical cancer screening.
