Healthy Community Coalition (HCC), an independent affiliate of Franklin Community Health Network (FCHN), is one of the oldest and largest coalitions in the country, recognized as a leader among its peers due to a reputation for innovative programming, population health outcomes, policy and environmental improvements, and capacity and infrastructure building. HCC’s innovative approaches have led to statewide and national recognition including being named a Finalist in the Robert Wood Johnson Culture of Health Prize.

HCC’s mission is to measurably improve the health and well-being of all people in Greater Franklin County and neighboring towns using a coordinated public health approach of education, promotion, and outreach. A comprehensive, community-based health promotion, education and outreach organization, HCC provides prevention activities throughout Greater Franklin County to support healthy lifestyles that prevent disease and improve quality of life. HCC’s emphasis has always been to reach out to those in the greatest need in the community: the un- and underinsured, low-income, seniors, and high risk populations for each identified need.

HCC’s overarching organizational goals are to 1) Decrease Cancer Incidence and Mortality; 2) Improve Cardiovascular Health; 3)Decrease Obesity; 4) Decrease Substance Misuse and Addiction; 5) Decrease Poverty; and 6) Increase Access to Health Services.
