About This Grant:

The Franklin County CRC Cancer Initiative aims to boost colorectal cancer screening and early detection in Franklin County. Led by Community Health Workers, the project will tackle barriers like transportation, support, and insurance to improve health outcomes for at-risk residents. Partnering with MaineHealth and Franklin Health primary care, we seek to streamline screening pathways and ensure quick access to care. Our goal is to reduce colorectal cancer mortality by increasing screening rates at Franklin Primary Care by 10% and enhancing overall community wellness.
