About This Grant:
Community Care Partnerships of Maine (CCPM) will partner with Sacopee Valley Health Center and 20 other not-for-profit healthcare member organizations to increase provider knowledge and skills on increasing equitable access to meet evidence-based guidelines. CCPM will partner with organizations to co-design and implement screening processes, workflows, provider-friendly templates, and two-way data and report processes. This information will be integrated into a comprehensive educational toolkit, alongside educational materials and information on evidence-based prevention and screening practices, and disseminated and discussed across the CCPM network via monthly provider networking meetings and communication publications.
