About This Grant:
Sacopee Valley Health Center will partner with Community Care Partnership of Maine (CCPM) and six other member organizations to increase breast cancer screenings using CCPM’s centralized Pre-Visit Planning process. This involves a centralized Medical Assistant (MA) completing pre-visit plans for adult patients, including those due for a screening. The MA reviews patient records to assess health indicators and due screenings, distributing plans to the care team before visits. Additionally, CCPM partners with organizations to implement screening processes and workflows, integrating them into an educational toolkit disseminated across the network via monthly meetings and publications.
