About This Grant:
This two year initiative will implement evidence-based strategies to increase colorectal cancer screening rates to over 80% across Knox County, Maine. Pen Bay Medical Center’s Clinical Care Manager will lead a team of Primary Care Practice Nurse Care Coordinators and support staff in an outreach project to deliver colorectal cancer prevention education and screening opportunities to 2,800 patients in our region. The primary care team will work together with others across the organization to establish and sustain an effective workflow, including standardized procedures, to achieve and maintain our desired colorectal screening rate above 80%.
