About This Grant:
Saint Joseph Hospital will partner with Community Care Partnerships of Maine (CCPM) and five other member organizations to increase preventative colorectal cancer screenings through CCPM's centralized Pre-Visit Planning (PVP) process. Through this project, the PVP will deploy a centralized Medical Assistant to complete comprehensive pre-visit plans for annual visits for adult patients, including those who are overdue for their colorectal cancer screening. Through patient record reviews, the Medical Assistant will assess chronic disease management health indicators, due or overdue preventative screenings and social determinants of health screenings.
