Lung Cancer Prevention and Early Detection Project

About This Grant:

St. Joseph Hospital will partner with Community Care Partnership of Maine (CCPM) – an Accountable Care Organization – and CCPM members Penobscot Community Health Center, Hometown Health Center, and St. Croix Regional Family Health Center to implement the Lung Cancer Prevention and Early Detection project. CCPM will develop standing orders and protocols for lung cancer screening referrals, smoking cessation education, and chronic disease management for patients diagnosed with COPD. CCPM will distribute and provide training on these resources to health care provider partners. Project partners will implement interventions with the targeted patients, address patients’ barriers to accessing care, including screenings, and centralize data analysis and project evaluation activities.
