Patient Navigation: A Pathway for Improving Health Outcomes

About This Grant:

Maine Primary Care Association and the Maine Medical Association Center for Quality Improvement are collaborating to strengthen clinic staff and expand patient navigation services in community health centers (CHCs) statewide. Using the American Cancer Society’s Leadership in Oncology Navigation model, the initiative will provide patient navigation training and foster a community of practice for newly trained navigators to share best practices and improve skills. Initially piloted with seven CHCs, the project will eventually expand to all 19 member centers, aiming to create a blueprint for reducing cancer incidence and mortality across Maine.
