Reducing Smoking Rates among Patients with COPD

About This Grant:

The Population Health Management of Waldo County General Hospital will develop, pilot, and evaluate a two-year RN- driven high-intensity personalized tobacco cessation intervention program to reduce smoking among patients of Waldo County Medical Partners who have a diagnosis of COPD. Grant funds will support hiring a part-time Tobacco Cessation RN Care Manager, who will work directly with patients, and their healthcare providers, to significantly decrease tobacco smoking rates among WCMP patients with COPD. The new program will support patients in accessing existing resources (such as the Maine Tobacco Helpline), while providing an intensive layer of face-to-face support and telephone counseling, monitoring, and follow-up.
