About This Grant:
Project TEACH is a comprehensive initiative to attain health equity for cancer patients of Aroostook County. By utilizing pre-existing services and community partnerships to serve as the foundation for this cancer care initiative, ACAP will leverage its proven methods of outreach and development to address the social determinants of health for our most vulnerable. The project will address transportation, education, access, care, and housing, ultimately increasing capacity for lodging and food, travel, and employing our "no wrong door" wrap-around service model that will connect patients and their families to all ACAP resources while they receive their treatments.
