About This Grant:
Harrington Family Health Center will utilize the population health module within its electronic health platform to create targeted outreach campaigns, send patient reminders, and identify unique barriers to cancer screenings. This work will be supported through analysis of patient-specific and provider-level data generated by the population health tool.
