About This Grant:
Greater Portland Health seeks to increase preventive cancer screening rates among populations most at-risk for breast, cervical, and colorectal cancers. Through our extensive experience providing culturally sensitive care to Greater Portland's underserved populations, we have found that community health workers (CHWs) have the unique ability to connect vulnerable community members with the resources they need to thrive via GPH's patient-centered medical home model of care. We currently have 6 CHWs who work to serve as cultural brokers and representatives of GPH within their respective communities, and we propose utilizing Maine Cancer Foundation funding to hire two new part-time CHWs to facilitate these vital screenings to individuals who need them most.
