About This Grant:
The Knox Clinic is seeking support to expand access to essential skin, breast, and colorectal cancer screenings by reaching community members who are not currently connected to primary care and bringing them into an ongoing, comprehensive system of care. We are increasing screening rates through a wrap-around approach that combines proactive outreach, strengthened care navigation and referral coordination, and robust patient education on the importance of preventive screening and early detection.
