About This Grant:
Community Care Partnerships of Maine will partner with Bucksport Regional Health Center to increase colorectal cancer (CRC) screenings, with a focus on younger-onset CRC (under age 50), through a comprehensive pre-visit planning (PVP) process and population health data analytics. The PVP process involves a centralized Medical Assistant (MA) who completes comprehensive pre-visit plans for adult patients’ annual visits, including those who are due or overdue for cancer screenings. PVP MAs distribute completed pre-visit plans to the local care team prior to patient visits to ensure a comprehensive appointment. CCPM will utilize a population health tool that integrates and analyzes information from disparate data sources to identify patients who are due or overdue for CRC screenings and incorporate screening recommendations into the pre-visit planning process.
