Pre-Visit Planning: Colorectal Cancer Screening

About This Grant:

Community Care Partnership of Maine, St. Joseph Hospital and 5 other member organizations will collaborate to increase colorectal cancer screenings. The centralized Pre-Visit Planning (PVP) process will deploy a Medical Assistant (MA) to complete comprehensive pre-visit plans for adult patients. PVP MAs will distribute pre-visit plans to the local care team, and CCPM will work with partners to ensure follow-up care for screenings. The goal of the PVP: Colorectal Cancer Screening project is to increase earlier detection and treatment for those at risk of developing colorectal cancer. According to the US CDC and Preventative Task Force, approximately 68% of deaths from colorectal cancer could be avoided if all eligible people were regularly screened.
