About This Grant:
At MaineHealth (MH) we plan to significantly improve our colon cancer screening rates by building out our colon cancer screening documentation into our Epic Shared Electronic Health Record (SeHR) using Information Services support and a dedicated Medical Assistant to retrieve and identify patient documentation and data for colon cancer screening. Once this data is entered into Epic, appropriate documentation is retained, reminders set up, and updates made periodically in perpetuity. We will pilot this project on two MMC Portland-based practices that serve vulnerable populations with low colon cancer screening rates (MMC Internal Medicine Clinic
and Family Practice).
