Maine Immigrant Patient Navigation Project

Maine Migrant Health Program was awarded a three-year grant for Patient Navigation in 2016.

The Maine Immigrant Patient Navigation Project aims to eliminate health inequities specific to cancer prevention, screening and treatment by supporting the integration of the patient navigation model into two federally qualified health centers that serve predominantly diverse New Mainer communities (in central and downeast Maine). These FQHCs are Maine Migrant Health Program and Community Clinical Services. Immigrants’ healthcare needs are often inadequately addressed or understood by traditional systems of care. By using Community Health Workers this project will create patient navigation capacity to address the myriad of barriers that rural and ethnic minorities experience in Maine.
