Ambulatory Nurse Navigator with emphasis on Lung Cancer Screening

Southern Maine Health Care was awarded a three-year, capacity-building grant for Patient Navigation in 2016.

For this project, Southern Maine Health Care will initiate a nurse navigator position to support the patients of our new Cancer Care and Blood Disorders offices as well as aiding in the development and implementation of an early detection lung cancer screening program. The position will focus on a multitude of functions including patient education, coordination of care, prevention outreach, and patient support. Additionally, the navigator will assist our medical oncology, pulmonology, and primary care departments in working to define and implement a screening initiative for lung cancer to reduce the burden of late stage diagnosis.
